ZeOmega Inc. Releases Jiva 5.6 Population Health Management Solution

ZeOmega Inc. announced the release of its Jiva™ 5.6 population health management solution with major enhancements that include advanced analytics, patient registries, dashboards, medication therapy management (MTM) and other data-sharing tools. The enhancements in the latest release are designed to reduce admissions, lower healthcare costs and improve the patient experience by helping healthcare organizations proactively identify and engage with patients most at risk for hospitalization. In particular, Jiva’s dashboards and patient registries will help organizations proactively achieve compliance with accountable care organizations (ACOs), CMS and other performance guidelines for improved care transitions and maximum reimbursement while supporting the best patient experience possible. The integration of medication therapy management (MTM) into Jiva’s overall patient care management process will help reduce avoidable and costly medication-related readmissions and emergency room visits. In addition, Jiva’s enhanced data sharing infrastructure enables clients to more seamlessly share data between Jiva and disparate clinical, administrative and financial systems in the healthcare ecosystem by using Jiva’s public application program interface (API). Jiva 5.6 also includes advances such as predictive analytics around risk for hospitalization, fully integrating medication therapy management with care management, and performance dashboards for proactive management of quality initiatives. The enhanced platform also enables patient self-assessments, which gives care coordinators better, real-time data that drives the level of patient engagement needed to achieve optimal outcomes. Jiva 5.6 drives deeper patient engagement by presenting more complete data about patients. Instead of only indicating a patient’s medication non-compliance, the rules and protocols in Jiva 5.6 identifies patients at risk for medication-related adverse events, empowers care teams to precisely understand the issues a patient is having with their medications, such as potential drug-drug interactions, or why a patient isn’t taking their medications and what can be done to get that patient back on a medication plan that will optimize their clinical goals of therapy. While many enhancements were made in Jiva 5.6, highlights include: Expanded patient registry to identify and stratify patients most at risk for hospitalization; Improved identification of population members at risk for high-cost care so they can benefit from care management; Team-based interactive tools for new care management models that are closer to the patient; Broadly integrated medication therapy management that shares data across traditional care silos; Dashboards for Clinical Quality Measures so health plans can optimize Star ratings, and ACOs maximize their Medicare Shared Savings Program. As a highly scalable, flexible and modular solution, Jiva is designed to benefit virtually any healthcare organization, from health plans to hospital systems and accountable care organizations. ZeOmega Announces Jiva 5.6 Release

ZeOmega Inc. has announced the forthcoming release of Jiva 5.6, the newest version of its health management platform. The release includes new features and functionality that further expand Jiva's already-robust capabilities to support successful accountable care and value-based health delivery models. Jiva 5.6, which is scheduled for general availability this summer, offers a comprehensive array of enhancements to help healthcare organizations deliver sophisticated population health management programs that efficiently enhance the value of health care services. This major release features a streamlined user interface that simplifies gathering of patient assessment data, new dashboards and registries, enhanced care transition capabilities, and streamlined connectivity with clinical data sources. Additionally, several technological infrastructure enhancements will enable Jiva to consume increasingly diverse and larger volumes of data, a capability that will become critical as more stakeholders use Jiva to drive care collaboration. Jiva 5.6's latest enhancements include: A set of provider dashboards that allow ZeOmega clients to examine provider utilization, clinical quality, and service quality. It also helps monitor overall performance and enables clients to direct providers on areas of improvement. An enhanced population healthcare management registry that is designed to, among other things, streamline and automate patient identification and stratification processes according to CMS qualification guidelines, improve gaps-in-care functionality, and identify and monitor high-risk admission/readmission patients based on proprietary algorithms within a powerful inpatient census view. Upgrades to the integration architecture that enable seamless connectivity with diverse clinical data sources. Upgrades to Jiva's Sentinel Rules Engine to enable rules-based predictive, retrospective and prescriptive analytics using EMR, self-reported and claims data. This will streamline clinical performance measurement and management across the care continuum. ZeOmega will demonstrate Jiva 5.6 in booth 343 at AHIP Institute 2014, June 11-13 in Seattle.
