As part of its clinical integration program, Mount Carmel Health Partners is implementing physician-directed quality improvement programs, focusing on improving patient care and clinical outcomes. All providers are accountable for delivering demonstrated value to patients, payers and the community. With MedVentive Population Manager, a physician and his or her staff can now identify patients with a chronic condition, such as diabetes, and automatically do outreach to this group of patients that helps to drive care compliance and improve their overall health.
