Montefiore Medical Center Collaborates with UnitedHealthcare to Enhance Care, Improve Outcomes and Reduce Costs

Montefiore Medical Center announced that it is collaborating with UnitedHealthcare to provide enhanced, coordinated care to more than 9,000 New York residents enrolled in UnitedHealthcare's employer-sponsored health plans, including Oxford Health-branded plans. The collaboration focuses on prevention, improved coordination and well-managed care. Montefiore will share in the generated savings with UnitedHealthcare and reinvest those funds back into the system to ensure patients continue to receive the right care at the right time in the right place.

Montefiore Medical Center to Develop the Bronx Behavioral Health Integration Project

Montefiore Medical Center has been selected to receive a $5.6 million Health Care Innovation Award from the Center for Medicare & Medicaid Services to develop the Bronx Behavioral Health Integration Project, a program that will integrate behavioral healthcare for children and adults in primary care settings. Montefiore's Bronx-BHIP is designed to increase the availability of vital behavioral services and test an innovative reimbursement methodology that has the potential of generating health system savings. The program is the first time the integrated behavioral and medical care model will be tested on such a large scale at an academic medical center serving a safety-net population. Bronx-BHIP focuses on identifying and providing care to individuals of all ages for a broad range of behavioral health conditions in primary care settings. The integrated care model will administer evidence-based, age-appropriate behavioral health screening and other services to Medicare, Medicaid and Children's Health Insurance Program beneficiaries, as well as members of several health plans, in selected Montefiore primary care practices. There are also web-based video components to the program to facilitate access to care. Patients with behavioral disorders will be treated by a team that includes a psychiatrist, psychologist or social worker and a care manager, who will collaborate with primary care providers to provide screenings, assessments, treatment and other patient follow-up. The reimbursement methodology that Bronx-BHIP will test is designed to help accelerate adoption of the program and support program sustainability by paying providers a fee for the early identification of behavioral health issues and the continued treatment and care coordination of patients while being held to a number of quality indicators. The long term goal is to demonstrate that patients with behavioral health disorders can be successfully treated in a primary care setting at a lower cost to the healthcare system.

Montefiore Medical Center Announces an Agreement with Simone Healthcare Development Group to Lease a New 11-Story, 93,000-Square-Foot Medical Building in Riverdale

Montefiore Medical Center, in partnership with WESTMED Practice Partners, Inc. announced an agreement with Simone Healthcare Development Group to lease a new 11-story, 93,000-square-foot medical building in Riverdale. Simone Healthcare Development has acquired the site and will build a highly advanced facility, working closely with Montefiore to suit its specific needs for providing healthcare to the Riverdale community. The new construction project is scheduled to start Nov. 1 and finish approximately 18 months later. Montefiore will provide one-stop health care, including urgent care, primary care and a wide variety of specialty services, cardiology, nephrology, ophthalmology, orthopaedics, cancer infusion and complete radiology services. WESTMED will provide administrative and technological support, along with operational and financial services. Simone Healthcare Development will use its healthcare real estate expertise and specialized design skills to construct a state-of-the-art building that meets Montefiore's patient-focused specifications for this location. The Riverdale site will further expand Montefiore's care delivery network in the Bronx. Montefiore and Simone Healthcare Development also are developing the first 'hospital without bed', a 280,000-square-foot building located at the Hutchinson Metro Center campus in the Pelham Bay section of the Bronx.
