Fairview Health Services, North Memorial Health Care and Medica Partner to Create Fairview and North Memorial Vantage

Fairview Health Services, North Memorial Health Care and Medica have partnered to create Fairview and North Memorial Vantage. In 2012, Medica was the first health plan in the region to align with a major health care system in the development of an ACO. That ACO, Fairview Health Advantage, has successfully reengineered the approach to health care and member service. The addition of North Memorial adds 26 primary care clinics in the northwestern and western suburbs and two additional hospitals, North Memorial Medical Center and Maple Grove Hospital. Fairview and North Memorial Vantage will provide access to more than 2,500 primary and specialty care physicians, and eight hospitals. Fairview and North Memorial Vantage is offered on Medica's private exchange, My Plan by Medica. It will be available for new and renewing groups beginning with Nov. 1, 2013, effective dates. Current Fairview Health Advantage members will also have access to North Memorial providers beginning Nov. 1, 2013.

HealthEast Selects Medica for Health Coverage

Nearly 12,000 benefits-eligible HealthEast employees and their dependents will receive their health coverage from Medica, the two organizations announced. The coverage becomes effective Jan. 1, 2014. HealthEast employees will choose to receive their care through Medica Choice Passport or Inspiration Health by HealthEast, an Accountable Care Organization (ACO) developed by Medica and HealthEast. HealthEast employees who choose Inspiration Health will have the opportunity to take advantage of several ACO features including: Free advanced wellness assessment with a nutrition & fitness specialist and a certified health & wellness coach ($200 value) for new members within first year of membership; Virtual sessions with a Ways to Wellness nutrition and fitness specialist at discounted rates. Services include weight management, personal training, Pilates training, health and wellness coaching, cardiac wellness programs, and special metabolic and exercise testing. Care Connection 24/7 care concierge providing nurse triage, appointment scheduling and prescription refills. Welcome call for new members from a Care Connection advisor who will help transition medical records and prescriptions from other providers and answer any initial questions a member may have. Single phone number that members can use to contact either Medica or HealthEast. Service advisors staffing the phone line can provide members with assistance from appointment scheduling to answering claims and benefit questions.

Medica Health Plans, Inc. and HealthEast Care System Partner on an Accountable Care Organization Option in Medica's My Plan by Medica(SM)

Medica Health Plans, Inc. and HealthEast Care System announced a collaboration on an accountable care organization (ACO) option in Medica's My Plan by Medica(SM). The ACO will be called Inspiration Health by HealthEast and is available for an effective coverage date of November 1, 2012. Under an ACO model, the provider and the health plan work together to improve the quality of care, manage the total cost of care and optimize the consumer experience. Inspiration Health by HealthEast includes the following features: Free advanced wellness physical with a nutrition & fitness specialist and a certified health & wellness coach ($200 value) for new members when they schedule an annual health exam with their HealthEast primary care clinician within six months of new membership; Virtual sessions with a Ways to Wellness nutrition and fitness specialist at discounted rates. The Ways to Wellness Program provides wellness services that address the well-being including nutrition, fitness, and self-care; provided by registered dietitians, certified personal trainers and certified health and wellness coaches; Innovative patient advisory forum providing members with an opportunity to have an active voice in shaping and creating new programs and services to support optimal health and well-being; Care Connections 24/7 care concierge providing nurse triage, appointment scheduling and prescription refills; Welcome phone call for new members from a Care Connection advisor who will help transition medical records and prescriptions from other providers and answer any initial questions a member may have; Single phone number that members can use to contact either Medica or HealthEast. Service advisors staffing the phone line can provide members with assistance from appointment scheduling to answering claims and benefit questions; Care transitions personalized support scheduling primary care follow up appointment before leaving a hospital or emergency department stay; and access to HealthEast's comprehensive services including: HealthEast Heart Care, The National Brain Aneurysm Center, CyberKnife cancer treatment, Kidney Stone Institute, Spine Center, Integrated Medicine and Bethesda.
